Late Fees: If payment is recieved more than 10 days after the due date, an additional $15 charge on each past due payment will be imposed under this agreement. If the student/sponsor fails to pay an installment within 60 days of the due date, JIKA has the right to demand payment in full. Any items returned by the bank as non-payable will incur a service charge: of no less than $30 .
Acceptance: This agreement shall be fully earned and non -refundable at the time of execution; provided, however, that this agreement may be cancelled by the student /sponsor with conclusive proof to JIKA for any of the following reasons:
1. The student/sponsor may cancel this agreement within three(3) business days of its execution and written notice of such election is delivered by registered mail or by hand to JIKA. Any tuition received will be refunded.
2. The sponsor/student may cancel and receive a prorata refund if the undersigned moves out of town and no facilities exist with in a 15 mile radius .
3. The student /sponsor dies or becomes totally disabled during the the term of this agreement.
4. JIKA closes a dojo and fails to provide alternative facilities within a 15 miles of the original location.
ATTENDANCE: The student / sponsor understands that failure to regulary attend class and utilize JIKA'S programs does not relieve the Student/ Sponsor of the obligation to pay this agreement in full.
TERMINATION: The enrollment of the student/ sponsor shall be subject to all rules and regulations established by JIKA. This agreement may be terminated by JIKA at any time if the sole opininon of JIKA the continued membership of the student /sponsor would, for disciplinary , health,or other reasons jeopardize the programs of JIKA. In no such event, however, shall JIKA be required to refund any portion of the above sum of monies. It is further understood that the class times and and or instructors may change at any time at the sole discretion of JIKA.
MEDICAL EXAMINATION: The Student/ Sponsor while accepting the fact that JIKA will offer training in as safe an environment as can reasonably be provided, realizes that karate involves rigorous training snd physical contact and accepts sole responsibilty for obtaining a a comprehensive medical examiniation from a certified phyisician to determine whether karate could possibly be hazardous to his or her health and agrees to maintain regular check ups thereafter.
