  The undersigned student and / or parent hereby makes application for membership into the

JAPAN INTERNATIONAL KARATE ACADEMY (JIKA) and in consideration of acceptance, agrees

To the following terms and conditions:

Release:

The undersigned student intends to train in the art of karate and it is understood by said

Student (and sponsor, if student is a minor) that physical contact between students and

teacher, as well as rigorous exercises are involved in such training: therefore,

The student/sponsor hereby expressly agrees to release and discharge JIKA, Its agents

employees, teachers, assigns and representatives from all claims, demands, causes of  action including, but not limited to, the following:  Liability for any injury to the student whatsoever, It is understood that some classes are taught by JIKA at locations or in facilities not owned or Operated by JIKA: however this agreement is solely between the student/sponsor and JIKA,   and such use of such facilities does not constitute endorsement by the facility or its owners and the student /sponsor hereby releases such parties from any liability hereunder.
